On September 17, 2023, at approximately 3:24 a.m., EDT, a CSX Transportation (CSX) mechanical department employee was fatally struck by a remote-control locomotive in Walbridge East Bound Yard, Walbridge, Ohio. The employee, while crossing an active yard lead track to lock out a track for an airbrake test, failed to adhere to CSX Operating Manual Rule 2100.4, which mandates stopping and looking both ways before crossing a track. Despite proper lighting and operational locomotive systems, the employee did not observe the oncoming locomotive. Investigations confirmed compliance with safety and equipment standards, and fatigue, drug, or alcohol use were ruled out as contributing factors.
